Instructor Agreement

 

 

This document/website page constitutes an agreement between yourself and Blenderfordental to become an instructor for the Blenderfordental modules. 

 

By submitting this form you agree: 

• to the Blenderfordental’s ‘Instructor Agreement’ Terms & Conditions. 

• to Blenderfordental’s general Terms & Conditions.

• that you are free to enter into an agreement, and that this engagement does not violate the terms of any agreement between you and any third party.

 

Furthermore,

• RELATIONSHIP OF PARTIES

  • You agree that you act as an independent instructor, and that you are not entitled to any employee benefits from Blenderfordental. As an independent instructor we ask you to act as an ambassador for Blenderfordental, without authority to make representations or agreements on behalf of Blenderfordental.

  • You may not sell the Blenderfordental modules as an ‘agent’, unless written consent is obtained. All sales enquiries must be referred to www.blenderfordental.com

 

• FINANCIAL

  • There is no remuneration paid to any Instructor by Blenderfordental. You are free to independently charge for your own courses.

 

• CONFIDENTIAL INFORMATION

  • All information provided by Blenderfordental to you is confidential and the property of Blenderfordental. You may only disclose information regarding tuition of the Blenderfordental modules in a teacher-student capacity. You shall not knowingly infringe any copyright, trade secret or other intellectual property right of any third party.

  • You may not make copies or distribute the Blenderfordental software, includes add-on files as well as video tutorials.

  • You may not duplicate passwords. You may not share your module Activation Keys with others.

• TERMINATION

  • Blenderfordental may terminate this Agreement with 30 days, with prior written notice to you, likewise you will provide 30 days’ notice to Blenderfordental, should you wish to terminate this agreement. Your obligations pursuant to the above mentioned confidentiality clause shall survive any termination of this Agreement.

  • Misrepresenting Blenderfordental may result in instant termination.  

 

 

• INDEMNIFICATION

  • You agree to indemnify and hold harmless Blenderfordental, any and all liability, on account of any claim, suit or action made or brought against Blenderfordental, arising from the services you provide by tutoring the Blenderfordental modules. 

  • You agree to indemnify and hold harmless Blenderfordental for any damages resulting from any act of negligence on the part of Blenderfordental or its employees, or resulting from any representations previously approved by or materials or information supplied to you by Blenderfordental.                                                                                                   

 

• GENERAL

  • You may provide tutoring and related services for Blenderfordental at any mutually agreed time and location. Please keep in touch, email info@blenderfordental.com. These include: dental courses, private tuition, exhibitions, social media, your website, youtube and any educational setting. 

  • In order to hold our reputation and to provide the best services possible, we ask you to be professional at all times when dealing with any Blenderfordental content.

  • It is your responsible for planning and implementing an appropriate program for the presentation of the Blenderfordental modules.

  • You will need to have purchased the module, using your own login credentials.

  • As an ambassador for Blenderfordental, you will need to include the Blenderfordental logo in your marketing material, ie, videos, pamphlets, social media posts. If you have your own website, you will need to include the Blenderfordental logo with a link to www.blenderfordental.com

  • To use the Blenderfordental trade mark name please stay within the guidelines as set out in the Guidelines page.  

 

 

• Accreditation Process

  • Please send us a video showing how you work through the module you are applying for, then use the Booking Service to finalise your Instructor Application.

  • We may ask you for an interview (in person, via Skype, TeamViewer, messenger, phone, or other means) to complete our screening process. All of our instructors must be able to demonstrate the Blenderfordental software with due diligence and to be able to problem solve, or answer any questions regarding the Blenderfordental software and installation.

  • Accreditation will be confirmed after we have had the opportunity to complete the application process. Submitting this form does not guarantee accreditation. 

 

 

• Assignment

  • The Instructor shall not assign any of his or her rights under this Agreement, or delegate the performance of any of his or her duties to another party or Instructor, without the prior written consent.

  • You may only teach the modules you have been accredited for. This depends on your 'Level of Accreditation'.

 

 

Guidelines for using the Blenderfordental Trade Mark Name

 

 

When creating a website, social media group or video please work within the following guidelines when using the Blenderfordental name and logo.

1. Websites - Use your own website name. URL's with the name www.blenderfordental.XXX  are generally associated with an official Blenderfordental site. Unless you have written consent to act as a Blenderfordental agent, please do not use this name. Blenderfordental is trademark name. Rather use eg. www.yourownwebsitename/blenderfordental.XXX    

2. Social media eg. Facebook, Instagram, YouTube, Twitter - Use your own social media page or group name. You may use B4D plus ( your own name ). Your own logo is the primary logo, the Blenderfordental logo is the secondary logo and smaller. The names B4D XXX or Blenderfordental XXX, where XXX is a country name is generally associated with an official site. Unless you have written consent to act as a Blenderfordental agent, please do not use this name.

3. Video tutorials for accredited instructors - Always include the Blenderfordental logo (either the B4D tooth or the full logo) into your video for brand recognition. You can include your own signature or logo as an accredited instructor to promote your business. The Blenderfordental logo with a country suffix may lead to misinterpretation as being an official site video. 

4. Promotional pamphlets, eg. course information - Use your own logo as a primary logo, the Blenderfordental logo as secondary and smaller logo. If you are an accredited instructor, please add the accredited instructor logo as well. 

5. Resellers - Please refrain from making your own videos and posting these. Video content can only be released by accredited instructors.

Privacy Statement

 

Blenderfordental will always adhere to current laws regarding Privacy. We aim to always hold secure the Privacy of all individuals we deal with. Your details will be stored in our management system and may only be used for advertising, promotional or alike services, with your prior consent. For B4D Instructors

 

By submitting this document you agree to these Terms & Conditions. The information requested on this application for you to become an instructor is necessary to ensure a fair and thorough evaluation your application with Blenderfordental. Personal information contained within this form shall be available only to employees and managers of the company with direct involvement in this recruitment process. You may update or access your application information at any time by contacting us at info@blenderfordental.com.

 

 

Submission of the ‘Instructor Application’ form indicates that you have read and that you agree to all terms of this Agreement.
